Practice Question of the Week
October 12, 2026: Simplifying Antiretroviral Regimens in Virologically Suppressed Patients With Drug Resistance
You are caring for a patient who has a history of virologic failure with known nucleoside reverse transcriptase inhibitor (nRTI) and nonnucleoside reverse transcriptase inhibitor (NNRTI) resistance. He has most recently been on darunavir (DRV)/cobicistat (cobi)/tenofovir alafenamide (TAF)/emtricitabine (FTC) + dolutegravir (DTG) with good tolerance, adherence, and viral suppression. Although this regimen is only 2 pills per day, he is very interested in something even simpler and with smaller pills. You discuss a variety of options, including bictegravir (BIC)/lenacapavir (LEN). Which of the following statements regarding the ARTISTRY-1 (Study to Compare Bictegravir/Lenacapavir Versus Current Therapy in People With HIV-1 Who Are Successfully Treated With a Complicated Regimen) trial is TRUE?
A. ARTISTRY-1 included people who were treatment naive without underlying HIV drug resistance
B. ARTISTRY-1 excluded those with any integrase strand transfer inhibitor (InSTI) resistance mutations
C. ARTISTRY-1 demonstrated that BIC/LEN is the best studied switch option for those who are virologically suppressed with highly nRTI-resistant virus
D. ARTISTRY-1 demonstrated that this patient would be less likely to remain suppressed if switched to a second-generation InSTI + FTC/TAF than if switched to BIC/LEN
